Nurses as primary providers of healthcare

Laura Haskins, MSN/MPH, and recent graduate at Johns Hopkins University, discusses her work as an Oncology Nurse Educator in Haiti and Rwanda through NGO Partners in Health. This work included leading the oncology training for nurses as well as quality improvement and research among other things. She described the importance of nurse educators and nurses in providing care in low resource settings around the world.
"Hi everyone, my name is Laura Haskins and I’m currently a graduate student at Johns Hopkins getting dual master's degrees in nursing and public health. Prior to the program, I was an Oncology nurse and after spending 6 years working clinically in the U.S. I spent 2 years working with the NGO Partners in Health as an Oncology Nurse Educator at the cancer centers that they support in Haiti and Rwanda.
While I was working there, one of my nursing colleagues told me that when she was in nursing school just a few years ago her professor told her that if you get cancer, you will die and that’s all you need to know about cancer.
And this is an extremely common view, particularly considering that public cancer care has only been available in these countries for less than a decade. But while the global the global health focus remains on communicable diseases, there is a growing burden of non-communicable diseases such as cancer and it’s really important that health systems and the healthcare workforce are strengthened to be able to meet these growing needs.
So one of my primary responsibilities as an educator was to teach the continuing and baseline oncology trainings for the nurses. We started with the cancer basics, went over chemotherapy mixing and administration, palliative care, and a lot more. But I also got to work at the more strategic and programmatic level doing things like research and quality improvement projects and even things like supply chain.
For me the most rewarding part of doing this work was getting to work alongside and mentor local nurse leaders. I truly believe that sustainability is the most important part of any global health work. Being able to see them grow and speak up as leaders on the healthcare team and knowing that they’re still doing that work today is extremely gratifying.
I learned so much from my nursing colleagues during this and every day they demonstrated the way in which nurses, particularly in low resource settings, are the primary people delivering care. Many tasks get shifted to them. They end up taking on a lot of extra responsibilities to fill in the gaps in a weak healthcare system. So, my biggest takeaway from this work, which is something that I hope to carry with me into any future work that I do, is that if we want to support quality healthcare around the globe, we need to invest in and support nurses."

"Hola a todos. Mi nombre es Laura Haskins y soy una estudiante de posgrado en Johns Hopkins obteniendo doble maestría en enfermería y salud pública. Antes del programa era enfermera de oncología y después de pasar 6 años trabajando clínicamente en los EE.UU., pase 2 años trabajando con la ONG Partners in Health como enfermera educadora de oncología en los centros oncológicos que apoyan en Haití y Ruanda.
Mientras trabajaba ahí, una de mis compañeras de enfermería me dijo que hace unos años cuando ella estaba en escuela de enfermería un profesor le dijo que quien desarrollaba cáncer, moriría y eso es todo lo que tenían que saber sobre él cáncer.
Y esta es una opinión muy común especialmente considerando que el cuidado público de cáncer solo ha estado disponible en estos países por menos de una década. Mientras que el enfoque global de la salud global sigue siendo en enfermedades transmisibles, hay una carga creciente de enfermedades no transmisibles como el cáncer y es muy importante que los sistemas de salud y la fuerza laboral de salud se fortalezcan para poder satisfacer estas necesidades crecientes.
Una de mis responsabilidades principales como educadora era enseñar las capacitaciones de oncología continuas y de base a las enfermeras. Comenzamos con los conceptos básicos del cáncer, revisamos la mezcla y administración de quimioterapia, los cuidados paliativos y mucho más. También pude trabajar a un nivel más estratégico y programático haciendo cosas como proyectos de investigación y mejoramiento de la calidad e incluso cosas como cadenas de suministro.
Para mí, la parte más gratificante de hacer este trabajo fue trabajar junto con las enfermeras líderes locales y orientarlas. Realmente creo que la sustentabilidad es la parte más importante de cualquier trabajo de salud mundial. Poder verlas crecer y hablar como líderes en el equipo médico y saber que aún están haciendo ese trabajo hoy es extremadamente gratificante.
Aprendí mucho de mis compañeros de enfermería durante este trabajo. Todos los días demostraban la forma en que las enfermeras, sobre todo en entornos de bajos recursos, son las personas principales brindando cuidado médico. Muchas tareas se les son transferidas. Terminan asumiendo muchas responsabilidades adicionales para llenar los vacíos en un sistema de salud débil. Por esto mi mayor conclusión de este trabajo, algo que espero llevar conmigo a cualquier trabajo futuro que haga, es que si queremos apoyar atención medica de calidad en todo el mundo necesitamos invertir en y apoyar las enfermeras."
